Jazelle R. Ortiz, infant daughter of James M. Ortiz, Sr., and Jacqueline Susan Kelly of Tamaqua passed away upon birth on Saturday April 16, 2016 at Lehigh Valley Hospital Salisbury Township, Pennsylvania.
In addition to her parents she is survived b her twin brother, Xavier Ortiz, Liam Ortiz, James M. Ortiz, Jr.; Paternal grandfather, Louis E. Ortiz, Sr.; Maternal grandparents, Ann Marie Thornby, and Scott Kelly; preceded in death by a brother, Antonio Ortiz.
